–Introduction Music Plays– Like all of the DPIs, the Twisthaler requires an appropriate amount of inspiratory force and patient effort, therefore it should be inhaled with a quick, deep breath to deliver the medication appropriately to the lungs. Let’s watch as Casey describes and demonstrates how to use the Twisthaler to Mrs. Ryan. Hello, Mrs. Ryan I’m Casey. Hi, Casey. I’m here today to show you how to use a new inhaler device, this one’s called a Twisthaler. Ok. The Twisthaler has Mometasone in it. Mometasone is an inhaled steroid. So this inhaled steroid is prescribed by your clinician. So let me show you today how to use this new inhaler device. Ok. So first thing you do is hold it by the pink base. As you hold it by the pink base, you’re going to twist the top off. So twist the white top counterclockwise, it’ll come off. You can just set that on the table. Now that loads the dose of medication in here so you don’t want to be leaning it either way. You want to keep it upright so the dose doesn’t spill out. Now because this is a small amount of dry, white powder, you don’t want to exhale or breathe into the inhaler device, because that will just gum up the medication. Ok. So you’re going to exhale away from the device. Next you’ll put your mouth all the way around the device and a sharp, deep, inhalation. Because it’s a small amount of white powder, you want a good, deep inhalation to get to your lungs. Ok. So exhale. Inhale. Hold your breath for 10 seconds, and then exhale. Good. Now if you need a 2nd dose, then what you would do is take the white cap and put it back on the device, twist it all the way back down until it clicks. Excellent. Do the same thing again. You hold the pink base, twist it off, and now that loads the 2nd dose, so you’re ready to go again. Now this one has a counter on the bottom of it that helps tell you how many doses you have left in here. It counts down to double 0 and the double 0 means you need a new inhaler device, and some more medication. Ok. Since this has an inhaled steroid in it, what I want for you to do is to make sure that you rinse your mouth with water after you use this inhaler device. So rinse your mouth out with water and spit it out. So don’t swallow the water, but spit it out. Ok. And lastly to take care of this, you don’t want to get it wet because it has a dry powder in it. You just take a cloth and wipe off the mouthpiece if you need to. Oh, Ok. So now you put that back on, and you may be wondering what this is. Yeah. This is a patient trainer device. So it reminds people that you need to take a big, sharp deep inhalation to get that medication just into your lungs where it should be. So it’ll make a nice kazoo sound. Oh. So now your medication won’t, but the trainer will. So you’re going to pretend like this is your Twisthaler. You exhale away, inhale. Excellent. So it tells you you had a good, nice, deep inhalation. Got it. Now can you show me how to use this device? Ok, no whistle? No whistle. Ok, I like this better. Ok, I’m going to exhale. Great. Then put the cap back on. And you’re ready to go again. Ok. Good. Any questions? I don’t think so, it seems pretty simple. Wonderful.
